MODEL POLICIES AND AGREEMENT
ELIGIBILITY /APPOINTMENTS / CANCELLATIONS / LATE ARRIVALS
Model Eligibility
All models must be over the age of 21. Models may not be pregnant nor breastfeeding as cosmetic neurotoxin, dermal filler, and laser procedures are contraindicated for these patients. Models may not receive treatment within 6 weeks of significant events such as, but not limited to, weddings and travel. We screen all patient models as best we can for treatment eligibility before every model appointment. With that in mind, the final decision on whether a patient model is eligible to receive treatment will be made during the in-person pre-treatment assessment by our capable providers at DCCM™. Patient safety is our top priority at DCCM™ and our providers will NOT perform or approve any treatment plan that is deemed unsafe or unsuitable for any patient for any reason. Final treatment plans are made by our providers in-house and are based on their knowledge and expertise in the field of medical aesthetics as well as our patient model’s aesthetic goals. Treatment plans may not be altered or changed without our provider’s approval. Models always reserve the right to refuse treatment, however they may not make any claims for compensation from DCCM™ or its affiliates if they choose to do so.
Violation of these or any of the policies set forth in this model agreement will result in the model being held responsible for the cost of the service provided at full price as well as their ineligibility for all future model calls with DCCM™, and loss of any injector fees paid for past and future model appointments.
Appointment Scheduling
DCCM™ Academy has had the honor of receiving grants two from Allergan and Galderma to further educational opportunities for our students. As such, our models have the unique opportunity to receive aesthetic treatments at a premium as all product costs are covered by these grants. Models are still required to pay an injector fee specified at the time of booking. Please note payments are required in full at the time of booking and model slots cannot be held before payment has been made. Payments must be made by credit card and a credit card is kept on file. Your signature at the bottom of this document confirms your agreement to this policy and gives us permission to store and charge your card in the event of a late cancellation or no show.
We cannot guarantee your appointment will not start late or run over the allotted time.
Sometimes our model calls run behind. It is important for our models to take this into consideration if they have time sensitive prior commitments after their appointment. We are only able to offer our discounted pricing on these model call procedures as we are a recognized educational institution. To protect the health and safety of every one of our models and offer the best and most comprehensive educational experiences to our students, sometimes we need to allow for extra time. If you have a schedule with limited availability, we encourage you to book a regular appointment with us during Delle Chiaie Cosmetic Medicine’s normal business hours so we can accommodate you. Models should plan on 2 hours for a scheduled appointment on a model day.
Late Arrivals and Cancellations
If you are more than 10 minutes late, we reserve the right to cancel your booking or cut your service short. This may result in your forfeiture of the injector fee and your ineligibility for all future model calls. Should you need to cancel, all model bookings are non-refundable and non-transferable. Please call the office at 603.783.1087 during business hours of Tuesday – Friday from 8:00am to 5:30pm or send an email to info@dccmacademy.com. Outside of business hours, you may leave a voicemail or send us an email.
Promotions and Referrals
Model call bookings and associated charges are exempt from any promotions, sales, package pricing and or referrals. You may not use gift cards as a payment method. Rewards programs such as Alle and Aspire cannot be redeemed or earned on model days.
Returned Checks & Insufficient Funds
Balances due to insufficient funds as well as due to late cancellation and no show fees will remain on your account and a 20% charge will be applied to any unpaid balances for every 30 days past due. We reserve the right to apply this charge to any credit card that you have on file with Surface Medical Esthetics, PLLC, d.b.a.: DCCM™ and DCCM™ Academy, without notice. All patients are required to keep a valid credit card number on file. No further appointments may be scheduled until the balance is paid in full.
Medical Changes
Please notify us of any medical or health changes at the time of each appointment, so we can safely treat you. Please note if you decide against treatment on the day of service your deposit is non-refundable and non-transferable.
Follow up Appointments
Follow-up appointments at the intervals specified are mandatory for all models or they will be ineligible for any future DCCM model calls and subject to pay the entire cost of the treatment at full price plus $500 plus an additional $400 “no-show” fee. Follow-up appointments are held in the office exactly 2 weeks after your model day from 9am-10am. Models will be seen by a member of non-clinical staff for pictures. If you believe a touch-up is needed, our clinical staff will review your case and administer touch-ups as necessary. To qualify for this touch-up appointment, models must come to their 2 week follow-up.
