POLICIES

Appointment Scheduling

A $100 deposit is required at the time of booking. This deposit amount will be counted as a credit off the cost of your treatment if used within 30 days of your initial consultation. You may book using any accepted payment method including card, cash, check, or a gift card but please note that appointments cannot be held without a deposit. Payments are required in full at the time of each treatment. We accept most major credit cards, debit cards, checks, cash, and gift certificates. All appointments require a credit card number to be kept on file. Your signature at the bottom of this document confirms your agreement to this policy and gives us permission to store and charge your card in the event of a late cancellation or no show.

Consultations

Consultations are required for all new patients and existing patients coming in for a new treatment. Consultation cost is $100

Late Arrivals

If you are more than 10 minutes late for your appointment, we reserve the right to reschedule your appointment or cut your scheduled service short. This may result in a charge for services rendered or a fee based on our no show/late cancellation policy.

Cancellations

All cancellations require a 48 hour notice. Notice can be given by calling the office at 603.783.1087 during business hours of Tuesday – Friday from 9:00am to 4:30pm or sending an email to info@thedccm.com. Outside of business hours, you may leave a voicemail or send us an email. Failure to cancel within 48 hours of your scheduled appointment will be considered a late cancellation or appointment no show and will result in the loss of your $100 appointment deposit plus an additional $400 fee.

Repeat Last Minute Cancellations or No Shows

Should you no show or cancel within the 48 hour period a second time, you may only book an appointment on the day of treatment and will be responsible for the full cost of your treatment at the time of booking. You will lose the full amount of the charge should you no show or cancel your appointment outside of the 48 hour window.

Promotions and Referrals

You may not combine any promotions, sales, package pricing and or referrals. You may not roll over any promotions, sales or package pricing and or referrals. Essentially what we are saying is you may not “extreme coupon” us.

Cancellation Fees

What if the cost of your appointment is less than $100? A $100 late cancellation or no show fee will be charged for all appointments regardless of the value of the service for which you are scheduled.

Returned Checks & Insufficient Funds

A $100 fee will be charged for returned checks and the balance of that day’s services will be charged to your credit card on file. An additional 20% charge will be applied to any unpaid balances for every 30 days past due. We reserve the right to apply this charge to any credit card that you have on file with Surface Medical Esthetics, PLLC, DBA: DCCM™, without notice. All patients are required to keep a valid credit card number on file. Balances due to insufficient funds as well as due to late cancellation and no show fees will remain on your account and a 20% charge will be applied to any unpaid balances for every 30 days past due. No further appointments may be scheduled until the balance is paid in full.

Packages

To receive package pricing, payment must be made at the time of the first treatment or package sale. Package pricing is nonrefundable, non-returnable, non-transferable, and may not be applied to other treatment areas. Packages expire one year from the date of purchase. Unused services by the date of expiration will be forfeited.

Gift Cards

Gift Cards, including electronic and mobile gift cards, are available in any denomination and do not expire as long as you have the physical gift card or email.

Gratuities

Gratuities are appreciated on facial services such as HydraFacials, dermaplane facials, ect. It is not expected for you to tip on medical services such as neurotoxin and dermal filler appointments.

Medical Changes

Please notify us of any medical or health changes at the time of each appointment, so we can safely treat you.

Returns

All sales are final, non-refundable, non-returnable, and non-transferable. This is non-negotiable.

STUDENT CANCELLATION POLICY

Course and Training Day Cancellations / Late Arrivals

All appointments and classes require a credit card number to be kept on file. There are no cancellations or refunds for training days, observation days, course dates, ect. Please be sure of your course dates and availability before you book. Students must submit their medical/nursing license to us 2 weeks prior to the registered class date or it will be considered a default and the student will be dropped from the class and are subject to adherence to our cancellation policy.

In the case of a late arrival, your training will be cut short which may result in your inability to participate in any live model injecting opportunities. Please arrive at your training on time and plan for any traffic or extra travel time to avoid this.

Payment for the course or training day is due in full on the day of booking or scheduling. We cannot hold your space until we have received payment and a valid copy of your medical license.

Cancellation Fees

Upon cancellation, students are not eligible for a refund for the selected course or training. All courses and training opportunities are non-refundable, non-transferrable, and cannot be rescheduled.

MODEL CANCELLATION POLICIES

Appointment Scheduling

DCCM Academy has had the honor of receiving grants two from Allergan and Galderma to further educational opportunities for our students. As such, our models have the unique opportunity to receive aesthetic treatments at a premium as all product costs are covered by these grants. Models are still required to pay an injector fee of $100 for cosmetic neurotoxin and $150 for dermal filler and laser procedures. Please note payments are required in full at the time of booking and model slots cannot be held before payment has been made. Payments must be made by credit card and credit card numbers are kept on file. Your signature at the bottom of this document confirms your agreement to this policy and gives us permission to store and charge your card in the event of a late cancellation or no show.

Late Arrivals

If you are more than 10 minutes late, we reserve the right to cancel your booking or cut your service short. This may result in your forfeiture of the injetor fee and your ineligibility for all future model calls.

Should you need to cancel, all model bookings are non-refundable and non-trnsferable. Outside of business hours, you may leave a voicemail or send us an email.

Model call bookings and associated charges are exempt from any promotions, sales, package pricing and or referrals. You may not use gift cards as a payment method. Rewards programs such as Alle and Aspire cannot be redeemed or earned on model days.

Follow up Appointments

Follow-up appointments at the intervals specified are mandatory for all models or you will be subject to pay the entire cost of the treatment at full price. Follow-up appointments are traditionally held in the office 2 weeks after treatment.
